Community nursing team

Finding a good care provider in the community isn’t difficult, once you know where to look.

Our nurses will be there, when no one else can.

Nobody likes to go into hospital and with growing pressures on NHS and local authorities across Scotland and Northern Ireland, we believe that it is more important than ever to facilitate access to high quality, clinical care at home and in the community.

At the Scottish Nursing Guild (SNG), we provide a specialist community service which offers nursing and care support for clients at home and in the community - where there are gaps in their current care provision or in setting up new community care packages. We work closely with NHS Health Boards, local authorities, case managers, solicitors and private clients throughout Scotland.

Our community nurses are ready to step in to support discharge planning to promote early discharge from hospital; help prevent or reduce hospital admissions enabling the client to remain at home; maintain staffing in established or new packages and to ensure client safety and satisfaction.

With over 20 years’ experience, SNG are committed to providing a fast, reliable, professional and most importantly trusted service.

What can our community service offer you?

  • A nurse-led team with full case management if required, including compiling individualised care plans and risk assessments with on-going support from Case Manager
  • Care support for as long as you need us whether this is for a few hours or days to 24/7
  • Office based staff contactable 24 hours a day, 7 days a week
  • Rapid response to facilitate early discharge or to avoid hospital admission
  • Highly skilled nurses to provide home based specialist care tailored to meet the specific needs of clients with a variety of complex care requirements
  • Ad-hoc staffing to cover shortfalls in existing shift rotas or provide interim cover during recruitment periods
  • Assistance in stabilising staffing in long term complex care packages
  • Palliative / End of Life nursing care to facilitate clients wishes to remain at home
  • Care provision for clients who require assistance with personal and/or social care support
  • Respite care to support the client either at home or away from home
  • Support for both adults and children
